INTAKE PROCEDURE STEPS For Regional Center Clients Only

  1.  A regional center service coordinator or a parent/primary caregiver will contact 365 to begin the intake process. We will interview the person contacting us regarding the client's needs to ensure that we can provide in-home service. 
  2. If we can meet the needs of the client, we will ask that authorization for service (POS) be sent to us from the service coordinator. Our agency number is HQ1492 for San Diego and HM1581 for Orange County, this is the number you will need to give to the service coordinator to start the process. The initial POS should be sent via email to admin@365respitecare.com so service can be started immediately.
  3. The enrollment application will be sent to the primary caregiver/parent via email for electronic signature or via mail (in special cases, the form can be dropped off during the home visit). 
  4. Once we receive the POS, we’ll schedule a home visit so our scheduler can come to your home and do the initial intake.
  5. If you have a preferred provider (PP)/caregiver such as granny, siblings or other relatives, we'll pay them to provide the service to your loved one.
  6. If no PP is available, one to three caregivers (depending on the staff availability) will be scheduled to be interviewed with both the parent/primary caregiver and the individual present via zoom.
  7. Once you’ve selected a caregiver you are comfortable with and they are comfortable with you, service may begin.
